dc.description.resumoEste trabalho consiste na apresentação da teoria da comunicação em psicanálise segundo D. W. Winnicott (1896-1971) e o estudo de sua aplicabilidade clínica às intervenções psicológicas breves e os concomitantes psicodiagnósticos com crianças, conforme exposto em Consultas terapêuticas em psiquiatria infantil (1971 ). Discutir-se-ão alguns conceitos fundamentais para o tema: criatividade originária, gesto espontâneo, existência psicossomática, verdadeiro e falso si-mesmo (self), valorização da experiência de mutualidade na prática clínica winnicottiana e seu conceito particular de saúde psíquica. Tais conceitos são originários de uma larga experiência analítica com crianças anti-sociais na época das evacuações na II Guerra Mundial, com crianças muito pequenas (com meses de idade), que Winnicott atendeu inicialmente na condição de pediatra desde os anos 20, e com adultos esquizóides e esquizofrênicos em estado de regressão à dependência. A contextualização histórica da obra de Winnicott foi determinante para que ele propusesse algumas alterações teóricas significativas à psicanálise tradicional (Freud e Klein), como os conceitos acima e variações na técnica psicanalítica de acordo com as necessidades dos casos clínicos: os manejos clínicos. Para citar apenas duas, o trabalho segundo a demanda descrito em Piggle: relato do atendimento psicanalitico de uma menina (1977) e as consultas terapêuticas em psiquiatria inrantil (1971). Esta dissertação reflete sobre a tese de que as consultas terapêuticas são um exemplo preciso da originalidade e da peculiaridade clínica de Winnicott na realização de intervenções breves infantis e psicodiagnósticos compreensivos psicanalíticos, segundo os moldes de consultas psicológicas. Esta pesquisa apóia-se sobre o princípio de que a teoria das pulsões na obra de Winnicott é um tema secundário e que, portanto, sua peculiar concepção do adoecer psicótico está estreitamente relacionada à do próprio existir humano". (Dias, 1995, 1998) Assim, utilizar-se-á a teoria heurística da ciência inspirada por Th. S. Kuhn como enfoque histórico-epistemológico do desenvolvimento da psicanálise, sustentando o abandono do complexo edipiano como problema nuclear da psicanálise winnicottiana e produzindo uma verdadeira revolução no interior da psicanálise (Loparic, 1997a, 1997b). Assim, presume-se que a natureza, a etiologia e as formas de tratamento dos distúrbios psíquicos na teoria winnicottiana são concebidos a partir de critérios originais, distintos das teorias médicas e psiquiátricas de sua época, além de apresentarem diferenças significativas com relação à própria psicanálise tradicional". (Dias, 1998) Herdeiro do método psicanalítico freudiano ou, melhor, do estudo da natureza humana, conforme Winnicott, suas constantes reformulações teóricas ganharam aplicabilidade e respeitabilidade através de suas investigações teóricas e prática clínica, ambos aspectos inseparáveis de um mesmo processo contínuo de investigação e tratamento. Por exemplo, seu estudo sobre as consultas terapêuticas, que serão o objeto específico deste trabalho. Uma vez lançadas as bases desta reflexão, examinar-se-ão novos conceitos subseqüentes: a natureza humana como tendência à integração, o processo de amadurecimento como fruto da tendência à integração concretizada pelo ambiente facilitador, as conquistasde integração como experiências pessoais, a relação mãe-bebê, mãe devotada comum etc. Esses conceitos são tidos como favorecedores das explanações da teoria da comunicação no interior da teoria do amadurecimento humano de Winnicott e de possíveis distúrbios nesse processo, acompanhados de seus critérios de classificação de acordo com suas etiologias. Discutir-se-ão alguns conceitos centrais à clínica winnicottiana e empregados na condução das consultas terapêuticas: o jogo e o brincar, o sonho, a regressão à dependência, a transferência, o manejo e a interpretação. Tais conceitos favorecem a discussão das variações técnicas propostas por Winnicott às consultas terapêuticas. Enfim, serão apresentadas as consultas terapêuticas tal como desenvolvidas por Winnicott ao longo de sua obra, focalizando seu campo de atuação, indicações, limitações. As consultas terapêuticas podem ser definidas sucintamente como uma possibilidade de intervenção psicológica rápida e pontual (de uma a três sessões), favorecida pela própria busca de ajuda do paciente, que coloca o psicoterapeuta na posição do objeto necessitado. Para que o analista possa reconhecer tal necessidade, deverá realizar uma avaliação psicodinàmica do paciente, de sua queixa e sintomatologia. Apesar das consultas terapêuticas estarem respaldadas pelo conhecimento psicanalítico, não podem ser consideradas psicanálise ou psicoterapia breve psicanalítica exatamente por não realizarem um trabalho exaustivo ou focal da transferênciae por não apresentarem contra-indicações no que se refere aos distúrbios e transtornos psicológicos. Serão apresentadas cinco consultas terapêuticas com crianças da cidade de São Paulo, estudando suas utilidades diagnósticas e terapêuticas, além de lançarem-se algumas reflexões sobre as mútuas implicações entre a clínicae a teoria winnicottiana e o campode trabalho criado a partir das consultas terapêuticas com criançaspor
dc.description.abstractThis work consists of a presentation of the communication theory in psychoanalysis, according to D. W. Winnicott (1896-1971) and the study of its clinical application in brief psychological interventions and the psychodiagnosis with children, as it was exposed in Therapeutic Consultations in Child Psychiatry (1971). Some fundamental concepts for the theme will be discussed: primary creativity, spontaneous gesture, psychosomatic existence, true and false self, the valorization of the experience of mutuality in winnicottian clinical practice, and its particular concept of psychic health. Such concepts have their origin in a large analytical experience with antisocial children during the World War evacuations, with very small children (months old), that Winnicott treated inicially as a pediatrician since the twenties, and with schizoid and schizophrenic adults in a state of regression to dependence. The historical context of Winnicott' s work was crucial in order to promote some significant theoretical changes to traditional psychoanalysis (Freud and Klein). The concepts above as well as the variations of the psychoanalytical technique according to the necessity of the clinical case (clinical handling) are examples of lhe changes mentioned above. Tow Winnicott's innovations in terms of handling can be quoted: lhe work according to the demand described in The Piggle: an account oi the psychoanalytic treatment oi a little girl (1971) and in Therapeutic Consultations in Child Psychiatry (1971). This work will discuss the thesis that the therapeutic consultations are a precise example of the originality and peculiarity of Winnicott's clinic, when performing brief interventions with children and comprehensive psychoanalytical psycho-diagnosis, considering the pattems of. psychological consultations. This research is based on the principIe that "the drive theory in Winnicott's work is a secondary" theme and therefore "its peculiar conception of psychotic sickening is strictly related to human existence itself (Dias, 1995, 1998). Therefore the heuristic theory of science inspired by Th. S. Kuhn will be used as a historical and an epistemological perspective of the development of psychoanalysis, supporting the abandonrnent of Edipus complex as a nuclear problem in winnicottian psychoanalysis (Loparic, 1997a, 1997b). This abandonrnent finally produced a true revolution in the interior of psychoanalysis. It is possible to presume that nature, etiology, and ways of treatment of psychic disturbances in Winnicott's theory are conceived "from original criteria, distinct from medical and psychiatric theories of its time, also presenting significant differences from traditional psychoanalysis itself' (Dias, 1998). Heir of the psychoanalytical method, or the study of human nature, according to Winnicott's own words, his constant theoretical changes and innovations conquered the respect and application throughout his theoretical investigations and clínical practice, which are inseparable aspects of the same and continuous process of the investigation and treatment. As an example of these innovations, it is possible to quote his therapeutic consultations, which wiIlbe the specific subject ofthis work ante built the basis for this reflection, new concepts will be examined: human nature as a tendency towards integration, the maturational process as a result of the tendency towards integration performed by the facilitating environrnent, the achievements of integration as personal experientes, the relation mother-child, the ordinary devoted mother etc. These concepts help the explanations of the theory of communication in the interior of Winnicott's theory of human maturation and possible disturbances in this process, together with their criteria of classification according to their etiologies. Some other central concepts to winnicottian clinic will be discussed. These concepts are used in the management of therapeutic consultations: playing, dream, regression to dependence, transference, handling and interpretation. Such concepts support the discussion of technical variations proposed by Winnicott. Finally, it will be presented the therapeutic consultations as developed by Winnicott throughout bis work, focusing its field of application, indications as well as limitations. The therapeutic consultations can be briefly defined as quick and precise psychological interventions (from one to three consultations), helped by the patient' s own search for assistance, which puts the psycho-therapist in the position ofthe needed object. In order to detect such necessity, a psychodynamic evaluation of the patient, bis demands and symptoms must be done. Although the therapeutic consultations are based on psychoanalytical knowledge, thev cannot be considered psychoanalysis or brief psychoanalytical psychotherapy, because of the lack of a transferencial work and for not presenting contra-indications in which concems psychological disturbances and disorders. Five therapeutic consultations with children from São Paulo will be presented and the therapeutic and diagnostic uses of this technique will be . studied.
